1. Is the survey required?  Yes, the survey is required to be completed from the state of Illinois and local ordinances.

2. Why is the survey required?  To ensure the water remains safe from cross connections and backflow. 

3. What does the survey entail? Depending on what type of water customer, it is 5-15 questions with pictures, complete the survey to the best of your ability.

4. What if I select the wrong answer? Continue with the survey and contact CCRA Professional Services if you selected an incorrect answer. 

5. What happens with the results of the survey?   Most water customers that complete the survey there is no further steps required, if there is any follow up required CCRA or the city will contact the water customer.
